What Is an SDP Independent Facilitator?
The Self-Determination Program (SDP) empowers individuals with developmental disabilities and their families to take control of their own services and supports. As an Independent Facilitator, SDPath guides you through every stage of the process — from initial planning to ongoing management.
We work with individuals ages 3 to 70, meeting families where they are and building a plan that reflects their unique goals, values, and needs.
